CDSMP (Chronic Disease Self-Management Program)

The Chronic Disease Self-Management Program is a program that teaches individuals with chronic diseases how to better manage their chronic illnesses to improve their health and over-all quality of life.

A Matter of Balance

A Matter of Balance is a program designed to reduce the fear of falling and improve activity levels of older adults living in the community, thus reducing the number of falls and injuries from falls.

Diabetes Self Management Program

Teaches the skills needed in the self-management of diabetes and to maintain and/or increase life’s activities.
